Responsibilities of the Health Worker/Practitioner

As a Health Worker or Health Practitioner at Mulungu Aboriginal Corporation, you will

• Provide health care case management and liaise with staff and external providers to coordinate client care and health outcomes for client.
• Establish, promote and model positive and supportive relationships with staff and clients.
• Visit clients in their home to provide health care case management and liaise with staff and external providers to coordinate client care and health outcomes for clients.

Qualifications and Skills

We are looking for qualified candidates with experience in the industry and excellent communication skills.  You will have 

• Certificate III or Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice).
• Be of Aboriginal and/or Torres Strait Islander descent
• Hold a Current Blue Card, Australian Federal Police Check - current within the last 4 weeks (Or ability to obtain prior to commencement)
• Hold a Current Drivers Licence
