Your Role as an Aged Care Connector

In the role of Aged Care Connector, you will be at the forefront of advocating for culturally safe aged care assessments and ensuring clients understand their entitlements within the aged care system. Your expertise will guide clients and their families through the complexities of aged care, helping them to access the services they need and deserve.

This dynamic position requires you to develop strong relationships with clients, care teams, and community members, all while maintaining accurate data and reporting. You will play a pivotal role in supporting Elders and their families, ensuring their voices are heard and their needs are met.

Who We Are Looking For

We are seeking a passionate and dedicated individual who is eager to make a difference in the lives of Aboriginal and Torres Strait Islander communities.

  • Minimum Certificate III in Community Services or equivalent, or willingness to obtain.
  • Experience working with Aboriginal and/or Torres Strait Islander communities.
  • Strong interpersonal communication skills with an ability to engage effectively with diverse groups.
  • Capability in case management, data collection, and quality improvement processes.
  • Hold a current Drivers Licence and Blue Card.

Your understanding of person-centered care and trauma-informed practice will guide your work as you support clients in a culturally respectful manner. If you are a self-starter with a knack for problem-solving and a commitment to community empowerment, we want to hear from you!
