Position Overview (Full Time / Part Time)
The Culture Care Connect Aftercare Worker will be responsible for delivering timely and culturally appropriate support to Aboriginal and Torres Strait Islander individuals who have experienced a suicide attempt or crisis. This role requires a compassionate approach, working closely with clients to ensure they receive the necessary care coordination and follow-up services.
As an integral member of our team, you will implement a co-designed service delivery model that prioritizes client empowerment and resilience. Your role will involve engaging with various community services to create strong referral pathways and contribute to the overall wellbeing of our clients.
Candidate Profile
We are seeking a dedicated professional with a strong understanding of Aboriginal and Torres Strait Islander cultures, mental health frameworks, and crisis intervention strategies.
- Certificate IV Community Services or Similar Experience in Mental Health / Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent experience.
- Proven skills in client advocacy and customer service.
- Ability to handle sensitive and confidential matters with discretion.
- Proficient in electronic medical software and Microsoft Office.
The successful candidate will demonstrate the ability to work both independently and as part of a multidisciplinary team, possess exceptional communication skills, and be committed to ongoing personal and professional development. Knowledge of Quality Improvement processes and contemporary Human Resource Management practices is also essential.
Selection Criteria
SC 1 Demonstrated knowledge and understanding of Aboriginal and Torres Strait Islander cultures and the social, economic, and cultural factors affecting Aboriginal and Torres Strait Islander people in today’s society.
SC 2 Demonstrated thorough knowledge and understanding of SEWB and Mental Health frameworks as well as best practice guidelines and treatment options for suicide prevention and self-harm risk.
SC 3 Demonstrated sound interpersonal and communication skills including the ability liaise and network with a range of stakeholders including internal and external service providers as well as effectively communicate with Aboriginal and Torres Strait Islander people of all ages.
SC 4 Ability to work both in a multi-disciplinary team environment, independently with limited supervision, prioritise and meet deadlines, manage matters of a sensitive and confidential nature and meet organisational demands.
SC 5 Demonstrated ability to implement and work within a case management framework to deliver services including the ability to undertake crisis intervention and work with individuals referred to the service.
