Mulungu Aboriginal Corporation Primary Health Care Service (Mulungu) is dedicated to improving health outcomes for Aboriginal and Torres Strait Islander peoples through comprehensive primary health care. Our organisation operates across three key areas: Health Services, Social Services, and Corporate Services, working collaboratively to deliver impactful support to the Indigenous community and ensure that health and wellbeing are prioritized.
Position Overview
The Elder Care Connector plays a vital role in assisting clients to navigate the My Aged Care system and access local aged care support services. This position is integral to enhancing the quality of care for Elders within the community, ensuring they receive culturally competent advocacy and support throughout the assessment process. The Elder Care Connector will foster relationships with My Aged Care Assessors and engage with community stakeholders to continuously improve service delivery based on local needs and strategies.
Candidate Profile
The ideal candidate will be a compassionate advocate for Aboriginal and Torres Strait Islander Elders, possessing strong interpersonal skills and a deep understanding of the cultural dynamics within these communities.
- Minimum Certificate qualification in Community Services or willingness to pursue.
- Demonstrated experience working with Aboriginal and Torres Strait Islander communities.
- Exceptional communication skills, both written and verbal, with a capacity to engage diverse stakeholders.
- Proven ability to manage case loads, document interactions, and maintain confidentiality.
- Advantage to have previous exposure to My Aged Care.
- Strong problem-solving skills and capacity to work independently and collaboratively.
- Current Drivers Licence / Working with Children Blue Card / Current National Police Check (within the last six months) or abilty to obtain prior to commencement.
The successful candidate will also demonstrate a commitment to ongoing professional development and quality improvement in service delivery, ensuring that all clients receive the highest standard of care in alignment with organizational policies and goals.
Selection Criteria
SC 1 Demonstrated knowledge, or the ability to rapidly acquire knowledge, of practices, polices and legislation associated with Aged Care sector.
SC 2 Demonstrated awareness of the principles of person-centered care and trauma-informed practice and an understanding of how to assist and support clients to access services and reduce service complexity and confusion.
SC 3 Well established interpersonal and communication skills including the ability to liaise and network with a range of stakeholders and articulate the interests of Aboriginal and Torres Strait Islander individuals.
SC 4 Demonstrated ability to work independently with limited supervision, together with the ability to work in a multi-disciplinary team environment, prioritise and meet deadlines, deal with matters of a sensitive and confidential nature and meet organisational demands.
SC 5 Demonstrated ability to implement case management approaches to service delivery including the ability to work with individuals and families with complex needs.
SC 6 Demonstrated computer literacy skills, including Microsoft Office applications and other client management systems.
