Mulungu Aboriginal Corporation Primary Health Care Service is committed to delivering life-changing primary health care to Aboriginal and Torres Strait Islander people. As an Aboriginal Community Controlled Organization, we focus on enhancing health outcomes and overall well-being for our communities.

Our organization encompasses three primary service tiers—Health Services, Social Services, and Corporate Services—working cohesively to ensure comprehensive care is provided across Mareeba and Tablelands and surrounding areas.

Position Overview

The Culture Care Connect Aftercare Worker plays a vital role in providing timely and culturally appropriate follow-up care for Aboriginal and Torres Strait Islander individuals referred to our services post-suicide attempt or crisis. This position emphasizes community engagement and support, fostering resilience and empowerment to promote a holistic sense of safety and wellness.

By contributing to a localized service delivery model, you will ensure that individuals receive the necessary support in alignment with our co-designed model of care.

Candidate Profile

We are seeking a compassionate and skilled professional with a strong commitment to the principles of Primary Health Care and a deep understanding of Aboriginal and Torres Strait Islander cultures.

  • Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent experience.
  • Demonstrated knowledge of Mental Health frameworks and best practices for suicide prevention.
  • Exceptional communication skills and the ability to advocate for clients.
  • Experience in a multi-disciplinary team and independently managing sensitive client matters.
  • Current Blue Card, Qld Drivers Licence & Federal Police Check (ability to obtain prior to commencement)

The ideal candidate will have well-developed interpersonal skills, the ability to build strong referral pathways, and a commitment to ongoing professional development. Experience in a cross-cultural environment is highly regarded.
